Headache and Migraine Treatment

Most headaches are not dangerous, but frequent or disabling headaches deserve proper assessment, and some warning signs need urgent care.

What is it?

Headache is one of the most common neurological complaints. Primary headaches, such as migraine, are conditions in themselves. Secondary headaches are caused by another problem, such as infection, high blood pressure in the brain or, rarely, bleeding.

Headache types

  • Migraine

Throbbing headache, often on one side, with nausea and sensitivity to light or sound; some people have an aura before it.

  • Tension-type headache

A pressing or tight band-like pain on both sides of the head.

  • Cluster headache

Severe pain around one eye, in attacks that come in clusters, with a watery eye or blocked nose.

  • Medication-overuse headache

Frequent headaches made worse by taking painkillers on many days of the month.

What symptoms can occur?

Pain may be throbbing, pressing or stabbing, and may come with nausea, vomiting, sensitivity to light or sound, visual disturbance or tiredness.

What are common causes?

Migraine often runs in families. Common triggers include missed meals, poor sleep, stress, dehydration, bright lights, hormonal changes and some foods. Overuse of painkillers can make headaches more frequent.

When should someone seek urgent care?

  • A sudden, severe headache that peaks within a minute
  • Headache with fever, stiff neck, confusion or a rash
  • Headache with weakness, numbness, speech difficulty or vision loss
  • Headache after a head injury
  • A new headache that keeps getting worse, especially after the age of 50

For these warning signs, call 108 or go to the nearest emergency department.

How is it evaluated?

Assessment starts with a detailed headache history (a headache diary helps) and a neurological examination. Scans such as MRI or CT are advised when warning signs or examination findings suggest a secondary cause.

What treatment options are available?

  • Identifying and managing triggers, sleep and lifestyle factors
  • Medicines to treat attacks, used carefully to avoid medication overuse
  • Preventive medicines when headaches are frequent or disabling
  • Treatment of any underlying cause
FAQs

Frequently asked questions

When should I see a neurologist for a headache?
If headaches are frequent, changing in pattern, not helped by usual treatment, or affecting work and daily life. Sudden severe headache needs emergency care instead.
Do I need a brain scan for my headaches?
Not always. Most migraines are diagnosed from the history and examination. A scan is advised when there are warning signs.
Can migraine be cured?
Migraine usually cannot be cured, but treatment can often reduce how often attacks happen and how severe they are.
